AMENDED 282 (c) Subcontractor Responsibility Certifications shall be executed by the respective 283 subcontractors on forms prepared by the City and contain the same information, 284 representations and supporting information required in Contractor Responsibility 285 Certifications, including verification of apprenticeship qualifications required by Section 286 2-1101.3(e)(11) for each trade or classification of craft workers it will employ on the 287 project. 288 289 (f) Subcontractor Responsibility Certifications shall be executed by a person having sufficient 290 knowledge to address all matters in the certification and shall include an attestation stating, 291 under the penalty of perjury, that all information submitted is true, complete and accurate. 292 293 (g) A subcontractor listed on a firm's Subcontractor List shall not be substituted unless written 294 authorization is obtained from the City and a Subcontractor Responsibility Certification is 295 provided for the substitute subcontractor_ 296 297 (h) In the event that the City determines that a subcontractor fails to meet the requirements of 298 this ordinance or is otherwise determined to be non -responsible, it may. after informing the 299 prospective awardce. exercise one of the following options: 300 301 (1) Permit the awardee to substitute a qualified, responsible subcontractor in accordance 302 with the requirements of this section, upon submission of a completed Subcontractor 303 Certification for the substitute and approval of the substitute by the City. 304 305 306 307 (2) Require the awardee to self -perform the work in question if the firm has the required experience, licenses and other qualifications to perform the work in question; or 308 (3) Disqualify the prospective awardee. 309 310 (i) In the event a subcontractor is disqualified under this ordinance, and the general contractor, 311 construction manager or other lead or prime contractor is unable to cure the defect either
