' ' <br /> <br />OPTIONAL CUSTOMER INTERCEPT SURVEY <br /> <br />Maxfield Research Inc. would contract with the firm 0fSally Zor/Ch & Associates; a St. Paul- <br />based company that specializes in conducting consumer intercept surveys. Maxfield Research <br />Inc. has worked with this firm on twa other retail/commercial projects,'Hi-Lake Shopping Center · <br />and Downtown Long Lake to. conduct the customer-intercept surveys. Sally Zorich &Associates <br />has worked for. regional and national retail clients such as Target, Campy Snoopy and Mall of <br />America to conduct on-site surveys. Tkey are highly-qualified to conduct this type of research <br />and we use them exclusively. <br /> <br />The company provides on-site or on-location intercept surveys including in-store interviews, <br />door-to-door interviews, pre-recruited' one-on-one interviews, polls, executive, medical and <br />agriculturalinterviews, telephone interviews, among others. They conduct interviews in stores, <br />restaurants, offices, airports, convention centers, mails, hotels, zoos, sports facilities, movie <br />theaters, farms, etc. anywhere that a project takes them. <br /> <br />SAMPLE CONTRACT LANGUAGE <br /> <br />The following paragraphs outline Maxfield.Research Inc.'s standard contract language. <br /> <br />Payment <br /> <br />All invoices are payable to Maxfield Research Inc. within fifteen (I5) days of receipt of an <br />invoice showing the work completed and the direct costs for expenses. A finance charge of one <br />and one-half percent (.1.5%) per month will be added to. the unpaid balance of each invoice not <br />paid within thirty (30) days. <br /> <br />Disclaimer <br /> <br />The objective of this research assignment is to gather and analyze as many market components <br />as is reasonable within the time limits and projected staffhours set forth'in this agreement. <br /> <br />We assUme no responsibility, for matters, legal in character.
