2. Before the claim is paid, the interested public officer shall file an affidavit with the <br />City Clerk which affidavit shall provide the following information: <br />a. The name of the officer and the office held by the <br />officer: <br />b. The contract price; <br />c. The reasonable value; <br />d. The interest of the officer in the contract; and <br />e. That to the best of the officer's knowledge and belief <br />the contract price is as low as or lower than the price at <br />winch the commodity or services could be obtained <br />from other sources <br />The League of Cities recommends the use of model forms to ensure that the City complies <br />with State law regarding this issue. The forms are attached for Council information. If <br />Council determines that the quote provided by Contractors & Consultants Inc. is the <br />company they wish to contract with for this work, the Council will need to pass a resolution <br />for the purposes of complying with Minn State Statute 471.87-89. <br />Funding: <br />The deductible amount for this claim ($10,000) will be funded from the Insurance Fund and the <br />balance will be covered by the City's property insurance with the League of MN Cities. <br />Recommendation: <br />Authorize staff to enter into a contract with for roof replacement of <br />kiosks/gazebos at Woodland Green Park, Solstice Park, and Elmcrest Park based upon Council <br />discussion. <br />Council Action: <br />Adopt Resolution No. 09 -XX Authorizing Staff to enter into a contract with for <br />Roof Replacement of Kiosks/Gazebos at Woodland Green Park, Solstice Park, and Elmcrest Park. <br />Reviewed by: <br />City Administi <br />CC: 08/25/09 <br />
