AC 150/5900-1B 1/31/79 <br />(D) The clearinghouse may request that the completed <br />application be submitted through the clearinghouse for another 30-day review <br />period. <br />(ii) Complete Application. Sponsors may submit the complete <br />application to the appropriate clearinghouses in lieu of the NOI. The <br />clearinghouse is allowed 60 days to review the application and provide <br />comments. <br />(3) Civil Rights. Sponsors should ask the clearinghouses to <br />request comments from the public agency charged with enforcing or promoting <br />state and local civil rights laws regarding the civil rights aspects of <br />the proposed projects. <br />(4) Timing of Notification. Sponsors are encouraged to submit <br />their N0I at the earliest feasible time. <br />(5) Application Submission to the FAA. In submitting the <br />application to the FAA, sponsors should include all comments and recommenda- <br />tions, including those concerning civil rights, received from the <br />clearinghouses, along with a statement of what consideration has been given <br />to the comments. If no comments have been received, the "No Response" box <br />of Item 22b on the SF 424 should be checked. <br />(6) Old Applications. Applications not submitted to or acted upon <br />by the FAA within one year after completion of clearinghouse review are <br />subject to further review upon request of the clearinghouses. <br />24. PROJECT INFORMATION (FAA FORM 5100-101). This portion of the application <br />consists of Parts II thru V of FAA Form 5100-101. Instructions for com- <br />pleting the various parts are contained on the back side of the form. The <br />following supplemental instructions are offered for completion of the forms. <br />a. Protect Approval Information - Part II. Only items 4 and 5 require <br />completion with all other items not being pertinent to the PGP or duplicative <br />of information requested in other parts of the application. Item 4 should <br />be used to indicate whether or not the requested assistance requires state/ <br />local approval. As a general rule, this item will be marked "Yes" in those <br />situations in which there exists a state or local law authorizing a public <br />or planning agency to act as an agent on behalf of the sponsor with or <br />without participating financially in the project. The terms and conditions <br />of the agency and the agent's authority to act for the sponsor should be <br />stated in an agency agreement, a copy of which should be submitted as an <br />attachment. If the proposed project requires inclusion in a Unified Work <br />Program (see paragraph 71), such inclusion and approval shall be identified <br />in Item 5. <br />14 <br />Chap 2 <br />Par 23 <br />
