This work program should also <br />include, particularly with respect to system planning efforts, a description <br />of the structure or methods to be utilized to assure the effort achieves full <br />and necessary coordination among the various planning and implementing <br />organizations involved in the area of the study. In addition to the above, <br />this section should contain a list of organizations, consultants, and/or key <br />personnel who are anticipated to work on the project and their respective <br />areas of responsibility. If the consultant has already been selected, a <br />biographical sketch of the firm's experience and qualifications together with <br />its key personnel should be included. <br />(4) Geographical Location. A brief description of the general <br />area served by the airport and, in the case of a system plan, a map of the <br />area under consideration should be provided. <br />(5) Justification for Force Account Work. If the sponsor <br />proposes to accomplish the project work with its own forces, or those of <br />another public or planning agency, this fact should be so stated in the <br />application. Approval of the use of force account work will be based on the <br />sponsor's assurances and the FAA review of the adequacy and competence of the <br />personnel available to satisfactorily accomplish the proposed planning effort. <br />Qualifications of key personnel anticipated to perform the specific tasks <br />involved in the force account work should be included in this section. <br />(6) Sponsor's Representative. Include in this section the <br />identity of the person representing the sponsor in regard to the application, <br />including his name, title, address, and telephone number. <br />d. Assurances (Part V). This part of the application contains specific <br />assurances with respect to certain facts and legal authorizations. It also <br />requires the sponsor to comply with policies, guidelines, and requirements <br />relating to specific laws, regulations, and excutive orders. The sponsor <br />should read this part carefully and understand the full implications before <br />signing the application. In addition to these assurances, a completed and <br />signed FAA Form 5900-8, Standard DOT Title VI Assurances, should be attached <br />to this part. Questions concerning these assurances should be addressed <br />to the appropriate FAA office, or preferably at the time of the preapplication <br />conference when and if scheduled. Under certain circumstances, FAA may <br />require additional documentation relating to the legal authority of the <br />sponsor, availability of funds,' or other information relating to the <br />assurances specified in this part. <br />25. ASSEMBLING THE APPLICATION. After the documentation has been prepared <br />and the content coordinated with the FAA field office, the documentation is <br />assembled into an application.
