<br />entered into between the City and the successful Proposer to provide the transit services <br />described in this RFP.] <br /> <br />1. Transmittal Letter <br /> <br />The proposal shall be accompanied by a transmittal letter signed by an officer or managing <br />principal who can bind the company to the offer presented and provide a personal <br />commitment to the success of the project. The transmittal letter must acknowledge receipt of <br />all addenda issued, by number. <br /> <br />2. Proposer Description <br />a. Identify the Proposer's organization, including: <br />- the complete legal name of the Proposer; <br />- the Proposer's complete corporate address; and <br />the name, address, phone number, e-mail address, and position of the Proposer <br />representative to be contacted regarding this proposal. <br />b. Describe the Proposer's organization, including: <br />- the number and organization of staff located in the Twin Cities area available <br />to support this project; and <br />- the number and organization of staff located elsewhere available to support <br />this project. <br />c. Describe how this contract will be managed if the Proposer does not currently <br />have and does not intend to establish a Twin Cities-based office during the term <br />of the contract. <br />d. Describe the Proposer's history, financial resources, capabilities and stability. <br />Include the previous three years of financial statements if a publicly owned entity. <br />If a privately held firm, include the firm's previous three years of income/expense <br />sheets and balance sheets for the end of each fiscal year and three years of <br />debt/equity ratios. <br />e. Provide information demonstrating the Proposer's ability to fiscally manage and <br />monitor contracts of the type to be entered into for the described transit services. <br />f. For any subcontractor proposed to work on this project, provide the following: <br />- the complete legal name of the firm, corporate address, and phone number; <br />a description of the subcontractor's organization, including number and <br />organization of staff in the Twin Cities area available to support this project, <br />and number and organization of staff in located elsewhere available to support <br />this project; <br />- the work that each subcontractor will perform on this contract; and <br />- an estimate of the percent of the work on this project the subcontractor will <br />perform. <br /> <br />3. Qualifications <br />a. Clearly and completely describe the Proposer's general and specialized <br />qualifications to undertake this project, both from the aspect of providing project <br />management of the assembled team and the aspect of providing the services <br />required for this project.
