If any subcontractors are proposed to be used in this project, clearly and <br />completely describe each subcontractor's general and specialized qualifications to <br />undertake this project. The proposal must demonstrate that all subcontractors to <br />be used have the qualifications to successfully prosecute and complete their <br />identified portion of this project. <br /> <br />4. Experience <br />a. Provide a description of the specialized experience and competence of the <br />Proposer and its personnel to provide the services of this project. <br />b. Provide records of Proposer performance on all transit service contracts from <br />2001 to the present date, including: <br />the contract name; <br />- client name, address and phone number; and <br />- the date the project started and was completed. <br />c. Provide the identical information in the previous two clauses for any <br />subcontractors proposed to be used on this project including the role of each <br />subcontractor on the projects described. <br />d. Provide a description of any defaults on previous contracts including a full <br />explanation of the circumstances surrounding the default. <br />e. Provide a description of quality of service issues on previous contracts including: <br />an explanation of any quality of service issues evidenced in the <br />performance of previous passenger transportation contracts, such as on- <br />time performance and responsiveness to user needs; <br />how such issues were resolved; and <br />challenges in performing the service and resolving customer complaints. <br /> <br />5. References: <br />a. For the Proposer: <br />Identify three references that the City may contact regarding past performance <br />on transit service contracts. Identification of each reference shall include: <br />· the contract title; <br />· client name and address; <br />· reference contact name and phone number; <br />· complete description of the work performed; and <br />· start and finish date. <br /> <br />Proposer references must be from entities who have contracted with the <br />Proposer and relate to experience in the passenger transportation industry, <br />particularly in regular route public transit or paratransit. Proposer references <br />will be asked to provide: <br />· a qualitative assessment of performance on previous and similar contracts; <br />· a list of contracts and scope of contracts held by the Proposer; <br />· information on any defaults on previous contracts; and <br /> <br />Instructions for Preparation of Proposals <br />Page 6 <br />
