For Subcontractors proposed to be used by the Proposer, the Proposer must <br />supply the same information as for Item 5(a), above, for each proposed <br />subcontractor. <br /> <br />6. Key Personnel: <br />Identify key personnel to be used on the project, and their experience and qualifications, <br />for Proposer and each subcontractor that will be assigned to work on this contract, <br />including: <br /> <br />name; <br />title; <br />company affiliation; <br />proposed responsibilities; <br />detailed description of experience; and <br />- summary of qualifications. <br /> <br />In addition, attach each individual's resume. Each resume must be a maximum of two <br />pages in length. <br /> <br />7. Start-up Plan <br /> <br />Provide the following information regarding the Proposer's start-up plan for the project: <br />hiring timelines and specific functions of staff to be hired; <br />an estimate of completion dates for important tasks; <br />an identification of facilities, materials and equipment to be purchased or leased; <br />a description of the Proposer's capability to have a fully trained work force by <br />contract start-up date; <br />the location of the Proposer's facilities and their suitability to meet the <br />requirements of transit operation; and <br />a description of the suitability and capacity of facility and equipment to be used <br />on this project. <br /> <br />8. Service Management and Delivery <br /> <br />Provide the following information regarding the Proposer's service management and <br />delivery of the project: <br />plans for coordination with the City for service delivery; <br />the Proposer's policies and procedures; <br />a demonstration of a detailed understanding of the required work and services; <br />and <br />the Proposer's complaint resolution policy, procedures and training. <br /> <br />Instructions for Preparation of Proposals <br />Page 7 <br />
