§ 8.30 shall prepare a sketch showing the dwelling unit or structure, the structure dimensions, an arrow indicating which direction is north, dimensions from two structure corners to the septic tank or septic tank and well. Said information further including the depth of' the system, address of the property and last known date of p,~nping shall be provided on forms provided by the City and shall be kept on file by the pl~rnbing inspector. Completed forms shall be dated and signed by the homeowner and the inspector if other than the homeowner. Subd. 4~ Non-Approved Systems. If upon inspection, it is discovered that any part of the system is not constructed or operating in accordance with the minimum standards of this chapter, the plumbing inspector or his desig~nated representative shall give written no~Scatlon of the defects. The owner/applicant shall be responsible for the prompt elimination of all de .~ects and no system shall be replaced in service u.util ali defects have been corrected or elimJnated. If such defects are not corrected, the system shall be declared a health hazard in accordance vzith §8.30.08. Subcl 5. 'Tr~n{ng and Certification- The pl~rnbing inspector or Iris appointed representative(s) shall participate in training courses for local inspectors as they are offered by state sad local agencies and shall become certified. A certification progr~r, has been established by the Minnesota Pollution Control Agency. Subd~ ~ Licensing. No person shall en~age in the business of or perform for others the service of soil and percolation testing and site evaluation, installing, renovating, repairing, or purnpLug and cleaning sewage treatment systems within l%~rnsey without first obtaining a license therefore. Instal3er licensees shall be authorized to construct, Lusts]], renovate, repair, pump, clean and othervcise m~ntaLu sewage treatment systems required in §8.30.04 Sub& 6 and §8.30.05 Subd. 2 above. P?np-hauler licensees shall be authorized to pump and clean sewage treatment systems. Soil scientist licensees shall be authorized to perform soil and percolation tests and site evaluations. A person desiring to obtain any of the licenses described in §8.30.05 Subd. 6. shall submit an application therefore to the adn~/nistrator, which describes his: business and authorization to conduct such services; education, training certification, registrations and experience wfith respect to such services; n~rnber of persons employed in performing such services; and such other information as may be reasonably required by the adn~Jnistrator. Prior to the issuance or renewal of a license, license application shall be required to demonstrate a -knowledge and understanding of this chapter. The City Council may refuse to issue a license to a_ny applicant for reasons relating to the above factors and for other good cause. Licenses issued' pursuant hereto shall be in effect for one year and shall be accompanied by the payment of a license fee as established by Council resolution. Licenses may be revoked or refused renew'a] for cause inc]udinf, but not limited to: practice(s) or inst~Pation(s) which are in violation of any provision of this chapter or of any permit issued pursuant hereto; refusal on the part of the licensee to correct defective work on a sewage treatment system which is instal]ed or renovated within this jurisdiction.
