SECTION 2, APPLICATIONS FOR DISABILITY BENEFITS. All applications for disability benefits shall be submitted to the Board of Trustees at a regular or special meeting of the board. The application shall be accompanied by or be in the form of a sworn statement or affidavit stating the nature of the accident or disability which is claimed to the basis for benefits applied for, together with the date or dates of commencement thereof and the duty in which claimant was engaged at the time of the commencement of such disability, and such other information as the association may from time to time require. Such application shall be accompanied by a statement of a duly licensed physician or surgeon who shall state the nature of the accident or disability, the cause thereof and the physician's prognosis. The association may require in addition to the report of the applicant's physician such other physical and medical examinations as may be necessary to determine any question in connection with such application. All applications for disability benefits shall be made by or on behalf of the applicant within 30 days after the disability commences and no disability benefits shall be paid for a period of more than 30 days before such application is made. SECTION 3, APPLICATIONS FOR PENSION BENEFITS. All applications for pensions shall be submitted to the Board of Trustees at a regular or special meeting of the board. Applications shall be verified by oath of the applicant and shall state the following: (a) The age of the applicant; and (b) The period or periods of service in and the date of retirement from the Fire Department of the City of Ramsey, Minnesota; and (c) The length of time the applicant has been a member of this association; and (d) Such other and further information as the Board of Trustees may require. SECTION 4, APPROVAL OF APPLICATIONS. No benefits or pensions shall be paid until the application therefore has been approved by a majority vote of the Board of Trustees. Decisions of the board shall be final as to the payment of such benefits or pensions. No person receiving a pension shall be paid any other benefits by this association. SECTION 5, YEAR OF SERVICE. For purposes of computing benefits or pensions payable under Articles VII and VIII, a year of service shall be defined as a period of 12 full months of active duty in the Ramsey Volunteer Fire Department, beginning on the date when the member became an active fire fighter in said fire department. If a member's period of active service has not been continuous, parts of years may be added together to compute full years. ARTICLE VII, SICK AND DISABILITY BENEFITS. SECTION 1, PERMANENT DISABILITY BENEFITS. If a member of this association shall become totally and permanently disabled, to the extent that a physician or surgeon, acceptable to the Board of Trustees, shall certify that such disability will permanently prevent said member from performing his/her duties in the
