SECTION 2, APPLICATIONS FOR DISABILITY BENEFITS. <br /> <br />All applications for disability benefits shall be submitted to the Board of Trustees at a <br />regular or special meeting of the board. The application shall be accompanied by or be in <br />the form of a sworn statement or affidavit stating the nature of the accident or disability <br />which is claimed to the basis for benefits applied for, together with the date or dates of <br />commencement thereof and the duty in which claimant was engaged at the time of the <br />commencement of such disability, and such other information as the association may from <br />time to time require. Such application shall be accompanied by a statement of a duly <br />licensed physician or surgeon who shall state the nature of the accident or disability, the <br />cause thereof and the physician's prognosis. The association may require in addition to the <br />report of the applicant's physician such other physical and medical examinations as may be <br />necessary to determine any question in connection with such application. All applications <br />for disability benefits shall be made by or on behalf of the applicant within 30 days after the <br />disability commences and no disability benefits shall be paid for a period of more than 30 <br />days before such application is made. <br /> <br />SECTION 3, APPLICATIONS FOR PENSION BENEFITS. <br /> <br />All applications for pensions shall be submitted to the Board of Trustees at a regular or <br />special meeting of the board. Applications shall be verified by oath of the applicant and <br />shall state the following: <br /> <br />(a) The age of the applicant; and <br /> <br />(b) <br /> <br />The period or periods of service in and the date of retirement from the Fire <br />Department of the City of Ramsey, Minnesota; and <br /> <br />(c) The length of time the applicant has been a member of this association; and <br /> <br />(d) Such other and further information as the Board of Trustees may require. <br /> <br />SECTION 4, APPROVAL OF APPLICATIONS. <br /> <br />No benefits or pensions shall be paid until the application therefore has been approved by a <br />majority vote of the Board of Trustees. Decisions of the board shall be final as to the <br />payment of such benefits or pensions. No person receiving a pension shall be paid any <br />other benefits by this association. <br /> <br />SECTION 5, YEAR OF SERVICE. <br /> <br />For purposes of computing benefits or pensions payable under Articles VII and VIII, a year <br />of service shall be defined as a period of 12 full months of active duty in the Ramsey <br />Volunteer Fire Department, beginning on the date when the member became an active fire <br />fighter in said fire department. If a member's period of active service has not been <br />continuous, parts of years may be added together to compute full years. <br /> <br />ARTICLE VII, SICK AND DISABILITY BENEFITS. <br /> <br />SECTION 1, PERMANENT DISABILITY BENEFITS. <br /> <br />If a member of this association shall become totally and permanently disabled, to the extent <br />that a physician or surgeon, acceptable to the Board of Trustees, shall certify that such <br />disability will permanently prevent said member from performing his/her duties in the <br /> <br /> <br />
