The City of Ramsey currently has a dental plan for employees and their families, but it is a <br />voluntary plan, The proposal for 1991 is for the City to pay 100% for single and family dental <br />insurance coverage for all regular full-time and regular part-time employees. ~. <br /> <br />Using the figures above the approximate cost of ~nis request is(~507, plus an administrative fee <br />of approximately $75 annually, totalling the cost to approximately $582. <br /> <br />LONG TERM DISABIUTY <br /> <br />The City of Ramsey does not offer a long term disability plan currently. The proposal for the <br />1991 contract is for the City to provide and pay the premium for a long term disability plan for <br />all regular part-time and regular fulPtime employees. <br /> <br />The cost of this proposal will be available in approximately two weeks. <br /> <br />SHORT TERM DISABILITY <br /> <br />The City of Ramsey does not currently offer a short term disability plan. The proposal for the <br />1991 contract is for the City to provide and pay the premium for a short term disability plan <br />for all regular part-time and regular full-time employees. <br /> <br />This plan is for 2/3 of monthly salary, for up to 6 months, coverage begins on 1st day of injury <br />or 8th day of an illness. <br /> <br />The cost of this proposal will be available at the meeting on September 5th, 1990. <br /> <br />pROPOSALg, LIFE INSURANCE <br /> <br />For the 1989 and 1990 contract the City's contribution of $195 per month is used toward <br />health insurance and $5,000 life insurance for employee only. The cost of the $5,000 is <br />$.85/month per employee or $204 annually. The proposal for the 1991 contract is for the <br />City to provide and pay the premiums for a group term life insurance with a $20,000 benefit <br />amount. <br /> <br />The cost of this request is $3.40/month per employee, or $816 annually. <br /> <br />PROPOSAL 1 Q, LEAVES <br /> <br />The proposal for the 1991 contract is the incorporate all current leaves under one Article. <br />This would include jury duty, military leave, leave of absence, etc. <br /> <br />There is no cost of the request. <br /> <br /> <br />
