be used by the City or other parties to design or construct any facility, in whole or in <br />part, without the further agreement of, and compensation to, the authoring party. <br />E. Proposal (RFP): <br />Proposers shall submit twelve (12) copies of their completed RFP response. All <br />Proposals must be limited to the following prescribed information and be submitted in <br />an 8 1/2" x 11" format and be in a comb binder, not submitted in a loose-leaf, three-ring, <br />hard cover binder. Pages are to be numbered in sequence and font size will be at least <br />11pt. font. If divider sheets are used, no information, written or graphic, other than the <br />section number corresponding to the following listing is to be included on the divider <br />sheet. Proposals shall not exceed 25 single -sided pages in length (double -side pages <br />count as two pages). This page limit does not include cover pages, divider sheets, or <br />required forms. <br />The List of Forms to be included with proposal includes: <br />Attachment 1- Consultant Disclosure Form XXXX <br />Attachment 2 - Joint Venture Eligibility Form Of applicable) <br />Attachment 3 - Joint Venture Affidavit (if applicable) <br />Attachment 4 - Consultant Team Members <br />Part 1 Letter of Transmittal. Clearly indicate the single contact (principal -in -charge), <br />mailing address, e-mail, telephone and facsimile numbers. <br />Indicate unique features of the organization and the project team that makes the <br />team uniquely suited to undertake this specific project. <br />Part 2 Key Personnel. Identify the individuals from each firm who will be involved in <br />the project and their responsibilities. Provide brief biographical data of the primary <br />participant(s), including the lead designer(s) and the Project Manager, if <br />different. Identify the organization and percentage of time for which the key <br />individuals of the team are committed to the project. Include major <br />subconsultants. <br />Part 3 Team Members. Identify the proposed subconsultants/subcontractors that will <br />be part of the team with a brief description of services they will provide. <br />Describe the capacity of each firm to do the work. State whether the firms have <br />previously worked together, and if so, in what capacity. <br />Part 4 Organization and Management of the project. <br />a. Describe how the project will be organized, both internal to the design <br />team and how coordination with the project stakeholders & MnDOT and <br />BNSF Railway will be undertaken. <br />b. For the project team, summarize what work the Consultant will self <br />perform and what work will be performed by sub consultants. <br />c. Describe the team's design and management approach to the project. <br />Describe how the team will accomplish the work in a thorough, effective, <br />and timely manner.
