Street addresses at which applicant and present spouse have lived during the preceding ten years; Whether applicant or his spouse has ever been convicted of any felony, crime, or violation of any ordinance other than traffic; if so, the applicant shall furnish information as to the time, place and offense for which convictions were had; g. Whether applicant or his spouse or a parent, brother, sister or child of either of them has ever been engaged in operating a saloon, hotel, restaurant, cafe, tavern, or other business of a similar nature; if so, applicant shall furnish information as to the time, place and length of time; h.' Whether applicant has ever been in military service; if so, applicant shall, upon request, exhibit all discharges; i.
