<br />Intoxicating Liquors Off-sale
<br />
<br /> a. The name and state of incorporation;
<br />
<br /> b. A true copy of the certificate of incorporation or association
<br /> agreement and bylaws;
<br />
<br /> c. The n~me of the manager or proprietor or other agent in charge
<br /> of the premises to be licensed, giving all the information about
<br /> said person as is required of a single applicant in Paragraph 2
<br /> above;
<br />
<br /> d. A list of all natural persons who, single or together with their
<br /> spouse, or a parent, brother, sister or child of either of them, own
<br /> or control an interest in said corporation or association in excess
<br /> of 5%, and who are officers of said corporation or association,
<br /> together with their addresses and all other information required
<br /> of a single applicant in subdivision 2 above.

<br /> Sub&& The exact legal description of the premises to be licensed
<br />together with a plot plan of the area showing dimensions, location of buildings,
<br />street access, parking facilities and the locations of and distances to the nearest
<br />church building and school grounds.
<br />
<br /> Sub& 6. The street number where the "off sale" sale of liquor is to be
<br />conducted. An applicant for An "off-sale" license shall submit a detailed blueprint
<br />plan of the building from which the liquor shall be sold.
<br />
<br /> Sub& 7. If a permit from the federal government is required by the
<br />laws of the United States, whether or not such permit has been issued, and, if so
<br />required, in what n~rne issued and the nature of the permit.
<br />
<br /> Sub& 8. The ~rnount of the investment that the applicant has in the
<br />business, building, premises, fixtures, furniture, stock in trade, etc. and proof of
<br />the source of such money.
<br />
<br /> Subd. 9. The n~rnes and addresses of all persons, other than the
<br />applicant, who have any financial interest in the business, buildings, premises,
<br />fixtures, furniture, stock in trade; the nature of such interest, amount thereof,
<br />terms for payment or other reimbursement. This shall include, but not be l~r-~ted
<br />to, any leases, lessors, mortgagors, lenders, lien holders, trustees, trustors and
<br />persons who have co-signed notes or otherwise loaned, pledged, or extended
<br />securi~' for any indebtedness of the applicant.

<br /> SubcL 10. The nmmes, residences and business addresses of three
<br />persons, residents of the State of M~unesota, of good moral character, not related to
<br />the applicant or financially interested in the premises or business, who may be
<br />referred to as to the applicant's character or, in the case where information is
<br />required of a manager, the manager's character.
<br />
<br /> Subcl- 11. Brhether or not ail relevant property taxes for the premises
<br />to be licensed have been paid and, if not paid, the years for which delinquent and
<br />the amounts which are unpaid. A certificate from the Anoka County Auclitor shall
<br />be attached to each application veri ~fying the property tax status.
