The committee member or members will be responsible for providing community workers and volunteers With instructions and Mn/DOT supplied information packets pertaining to proper . materials handling, plant installation and plant maintenance.. 'If volunteers are to be used , .the._ application must include a volunteer pledge list identifying sufficient numbers of volunteers to complete the scope of work within an acceptable time period. The volunteer pledge list will also be a condition for award of the partnership project. The application must include a formal resolutioff'or letter indicating community support for the desired improvements and authorization of a primary ' ..: contact person to apply to Mn/DOT for funding. (See Appendix I for sample resolution or-letter): A Mn/DOT approved (District and Central Office) landscape plan at a scale of not more than 100;=i" must be included as part of the application (11" x 17" reductions are acceptable for applicati'bn purposes). The landscape plan shall incorporate by reference or attachment; standard:Mn/DOT ' - · planting details, material specifications and special provisions as applicable ........................... The plan may be prepared by Mn/DOT's Landscape Unit with no. obligation; as the Unit's scheduled ..::-.: work load permits. Conm'mnities desiring technical assistance or a plan :prepared ,by: Mn/DOT's.. Landscape Unit must submit a written request for that,service to the MrdDOT District:En~neer.: .. . -.- Potential applicants are encouraged to enlist the services of local community landscape design professionals to prepare plans, thereby minimizing time delays that may occur as a result of the Landscape Unit's workload. Mn/DOT's Landscape Unit will help the applicant to assemble base information and to include Mn/DOT policies, guidelines and standards. The Landscape Unit will also facilitate required Mn/DOT review and approvals. Funding for this program is limited to the cost of providing the required soil amendments, herbicides, plant materials, seed, sod, mulch, rodent guards and other miscellaneous materials necessary for installation of the project. The program requires that a community provide matching funds or services for every dollar of state funds or materials provided on state highway right of way. The partnership program considers the value of state funded or furnished landscape materials to be roughly equal to the value of the material installation by the partner._: The community must agree to install and maintain state funded or furnished materials. Eligible projects must include a.minimum of $1,000: ~worth~:0f_ materials and the maximum award of funds or materials for any one project shall not exceed $50,000 on an annual basis, Applicants may utilize their own labor~ forces, service organization, .volunteers .and contractors, however the local unit of government must be the contract-lettingauthoritywhere contract installation is desired. Volunteers must be 13 years of age or older unless accompanied and supervised by a parent or guardian. Successful applicants are required to enter into an Agreement with Mn/DOT;-.to maintain the roadside ....... landscape improvements for their anticipated lifespan. Maintenance will include removals and rePlacements if necessary. A detailed landscape plan showing actual locations and quantities'of plant ·: :: materials along with the specifications and special provisions will form the basis of the Agreement together with the application and schedule of maintenance requirements. The applicant must submit two copies of the application to the Mn/DOT District Engineer for review purposes. Each copy of the application must contain an original signature by a primary contact person.
