- 3 - <br /> <br />the removal, crushing, borrowing, filling and excavation of <br />material. <br /> <br />The costs to the City of the continuous adminstration of any <br />conditional use permit issued herein shall be paid by the <br />permit holder and will be billed monthly. These costs are in <br />addition to any security requirements of the conditional use <br />permit. <br /> <br />170.017M(5) Administration: <br /> <br />Application for the permit shall be made in writing to the <br />Council on such form as the Council may from time to time <br />designate, and shall include the following information: <br /> <br />Names and addrress of the applicant, operator and owner <br />of the land. <br /> <br />b. The purpose of the permit. <br /> <br />The exact legal description and acreage of the property <br />to be graded, mined or filled. <br /> <br />The following maps of the entire site and including all <br />areas within 350 feet of the site. All maps shall be <br />drawn at a scale of 1 inch to 100 feet unless otherwise <br />stated below: <br /> <br />Map 1 -Existing conditions to include: <br />- contour map (2 foot intervals); <br />- existing vegetation; <br />- existing drainage and permanent water areas; <br />-existing structures; and <br />- existing wells. <br /> <br />Map 2 - Proposed operations to include: <br />- location of sites to be graded, mined or filled <br /> showing elevations of each stage of proposed <br /> operations; <br />- location of storage of mined materials, showing <br /> maximum height of storage deposits; <br />- location of vehicle parking, access roads and hauling <br /> routes; and <br />- location and description of erosion and sediment <br /> control structures. <br /> <br />Map 3 - End Use Plan to include: <br />- final grade of proposed site showing elevations and <br /> contour lines at two foot intervals; <br />- location and species of vegetation to be planted; <br />- phasing plan; and <br />- snorm water drainage plan. <br /> <br />e. A soil erosion and sediment control plan. <br /> <br /> <br />
