The costs to the City of the continuous adminstration of any conditional use permit issued herein shall be paid by the permit holder and will be billed monthly. These costs are in addition to any security requirements of the conditional use permit. 170.017M(5) Administration: Application for the permit shall be made in writing to the Council on such form as the Council may from time to time designate, and shall include the following information: Names and addrress of the applicant, operator and owner of the land. b. The purpose of the permit. The exact legal description and acreage of the property to be graded, mined or filled. The following maps of the entire site and including all areas within 350 feet of the site. All maps shall be drawn at a scale of 1 inch to 100 feet unless otherwise stated below: Map 1 -Existing conditions to include: - contour map (2 foot intervals); - existing vegetation; - existing drainage and permanent water areas; -existing structures; and - existing wells. Map 2 - Proposed operations to include: - location of sites to be graded, mined or filled showing elevations of each stage of proposed operations; - location of storage of mined materials, showing maximum height of storage deposits; - location of vehicle parking, access roads and hauling routes; and - location and description of erosion and sediment control structures. Map 3 - End Use Plan to include: - final grade of proposed site showing elevations and contour lines at two foot intervals; - location and species of vegetation to be planted; - phasing plan; and - snorm water drainage plan. e. A soil erosion and sediment control plan.
