HF100 NINTH ENGROSSMENT REVISOR BD 1-10100-9 <br />108.1 (ii) the patient's acknowledgment that enrollment in the registry program is conditional <br />108.2 on the patient's agreement to meet all other requirements of this section; and <br />108.3 (5) all other information required by the Division of Medical Cannabis. <br />108.4 (b) As part of the application under this subdivision, a patient must submit a copy of a <br />108.5 certification from the patient's health care practitioner that is dated within 90 days prior to <br />108.6 the submission of the application and that certifies that the patient has been diagnosed with <br />108.E a qualifying medical condition. <br />108.8 (c) A patient's health care practitioner may submit a statement to the Division of Medical <br />108.9 Cannabis declaring that the patient is no longer diagnosed with a qualifying medical <br />i o8. i o condition. Within 3 0 days after receipt of a statement from a patient's health care practitioner, <br />108.11 the Division of Medical Cannabis must provide written notice to a patient stating that the <br />108.12 patient's enrollment in the registry program will be revoked in 30 days unless the patient <br />108.13 submits a certification from a health care practitioner that the patient is currently diagnosed <br />108.14 with a qualifying medical condition or, if the patient is a veteran, the patient submits <br />108.15 confirmation that the patient is currently diagnosed with a qualifying medical condition in <br />108.16 a form and manner consistent with the information required for an application made pursuant <br />108.17 to subdivision 3. If the Division of Medical Cannabis revokes a patient's enrollment in the <br />108.18 registry program pursuant to this paragraph, the division must provide notice to the patient <br />i o8. i 9 and to the batient's health care bractitioner. <br />108.20 Subd. 3. Application procedure for veterans. (a) The Division of Medical Cannabis <br />108.21 shall establish an alternative certification procedure for veterans who receive care from the <br />108.22 United States Department of Veterans Affairs to confirm that the veteran has been diagnosed <br />108.23 with a qualifying medical condition. <br />108.24 (b) A patient who is also a veteran and is seeking to enroll in the registry program must <br />108.25 submit to the Division of Medical Cannabis an application established by the Division of <br />108.26 Medical Cannabis that includes the information identified in subdivision 2, paragraph (a), <br />108.27 and the additional information required by the Division of Medical Cannabis to certify that <br />108.28 the patient has been diagnosed with a qualifying medical condition. <br />108.29 Subd. 4. Enrollment; denial of enrollment; revocation. (a) Within 30 days after the <br />108.3o receipt of an application and certification or other documentation of a diagnosis with a <br />108.31 qualifying medical condition, the Division of Medical Cannabis must approve or deny a <br />108.32 patient's enrollment in the registry program. If the Division of Medical Cannabis approves <br />108.33 a patient's enrollment in the registry program, the office must provide notice to the patient <br />108.34 and to the patient's health care practitioner. <br />Article 1 Sec. 52. 108 <br />
