E. FO~4 OF APPLICATION NOTE: kll applicants nust be on the official forms jointly provided herein by the cities of Anoka, Champlin, Ramsey, and Andover, Minnesota. The application forms have been designed to furnish all the pertinent data that will be used by the cities and C~,'~ssion in making their evaluations in accordance with the rules of the Minnesota Cable Cu~'~unications Board. Specific instructions for each form are stated on th~ individual form. Space has been provided for specific and succinct answers to all questions and requests for data. All applicants must use only th~ pages of the official application for~% (or identical extension pages if more roc~ is needed). Alternative proposal forms are neither desired nor will they be considered. Any attempt to merely use t_he official forms as an "index" to other voluminous d~ts may disqualify the applicant from consideration. The official forms have been designed to facilitate cu,~3rison of proposals. Evasive, imprecise, or incc~plete responses can only serve to the disadvantage of the applicant. Alternative proposals for services within a single application which, in effect, constitute a separate application will not be considered and may cause rejection of the entire application. (This does not prohibit the applicant frc~ assigning channels to levels or tiers of service with differing monthly subscriber rates). The cities and Cc~m~ssion reserve the right to reject any and all applications. F. CLARIFICATION OF APPLICATION ~S · In the event that any applicant may have any doubt as to any terms, conditions, or provisions of these specifications or the meaning or interpretation thereof, the applicant may request information or clarification thereon by sukn~tting such request in writing to: Anoka/Champ 1 in/lqam~ey ~ahle City of (~%amplin 12001 Highway 52 Champlin, F~ 55316 (612) 421-8064 Such requests for information must be su~uitted no later than May 22, 1981. A written response to such request will be made as soo~ as possible and will he sent to all known applicants who have been supplied applications. Only this type of official response shall be binding upon the cities and C~L~ssion. The applicant, by sut~aitting its proposal and response to the P~queSt for Proposal, shall have evidenced the fact that it agrees that it has no u~answered questions with respect to these specifications and shall have no basis for withdrawal or modification of its proposal on the basis of misunderstanding · e
