Change Order No 1 <br />Date: May 3, 2006 <br />Project: 05-23 Ebony Street Sewer Water and Street Improvements <br />Owner: City of Ramsey <br />Contractor: W B Miller Inc. <br /> <br />Contract Modifications: <br /> <br />Realign Pipe into Catchbasin No 4 <br />Description of Change: This project was bid with Catchbasin No 4 needing to be <br />reconstructed three feet to the west because of the relocation of the curbline. This existing <br />catchbasin had an additional lead connected into it from a storm project that was <br />performed by the Superbowl. This additional lead needed to be reconnected into the <br />relocated catchbasin. <br />Justification of Change: This work was needed to be done to have the storm sewer <br />function properly and provide an outlet for the storm water ponding system of the <br />Superbowl. The additional work was not anticipated at the time the contract documents <br />were prepared. <br />Cost of Change: The cost of this additional work is 2 hrs of crew time ~ $600 per hr or <br />$1,200. <br /> <br />Removal of Buried Structure <br />Description of Change: During the course of excavating for the installation of the water <br />quality treatment structure on 137th Lane, a buried manhole structure was discovered <br />behind the curb adjacent to an electrical transformer box. It was necessary to remove, <br />dispose, and sheet next to the electrical box. <br />Justification of Change: This work resulted from a change in anticipated conditions <br />Cost of Change: The cost of this additional work is 2 hrs of crew time ~ $600 per hr or <br />$1,200. <br /> <br />Relaying of CBMH No 7 <br />Description of Change: The plan sheet had an error which reversed the inverts of the <br />southeast inlet and the west inlet. This was not discovered until the manhole had been set <br />and several lengths of pipe had been layed in the field. As a result it was necessary to <br />reorder a corrected section of manhole barrel and relay pipe. <br />Justification of Change: The error was in the plans and not the responsibility of the <br />contractor. <br />Cost of Change: The cost of this additional work is 4 hrs of crew time ~ $600 per hr or <br />$2,400. In addition the cost of restocking the section of manhole barrel was $1,537 for a <br />total cost of $3,937. <br /> <br />38 <br /> <br /> <br />
