Remove and Dispose of 24 inch Concrete Pipe
Description of Change: The project inspector directed the removal and disposal of 48
feet of 2Z[ inch reinforced concrete pipe wtfich crossed north to south across 137th
Avenue.
Justification of Change: This was existing storm water pipe which is no longer required.
Removal was ordered to prevent material from migrating into the pipe and possibly
causing settlement of the road.
Cost of Change: The cost of this additional work has been requested as 48 feet of
removal and disposal at $I0 per foot for a total cost of $480.

Crane Rental Due to Modification of Water Quality Treatment Structure
Description of Change: The single unit system installed was heavier than the bid unit
and required a crane rental for installation.
Justification of Change: The plans called for the installation of a water quality
treatment structure to be placed in 137th Avenue and specified tkree acceptable
manufacturers or equal for the water quality structure. Because of the congested nature of
this intersection with other existing underground utilities it became apparent that only a
single structure type of water quality treatment unit could be installed without costly tight
sheeting or installation safety implications. The unit bid consisted of a two manhole
treatment system of which each component manhole could have been installed with the
contractor's equipment.
Cost of Change: The cost of the crane rental was a lump sum amount of $I,300.

Original Contract Price:
Previous Change Orders:
Change in Contract Price due this Change Order:
Total Contract Price including this and previous change
Orders:

$247,795.05
NA
$8,117.00

$255,912.05

This document will become a supplement to the conti'act and all provisions will apply

hereto.

Recommended:

Accepted:

Engineer Date
Contractor Date

39
