Defined contribution <br /> <br />applicant to the benefits of the workers' compensation act, Minnesota Statutes, <br />Section 176.011, Subd. 11, such report shall contain a statement whether or not, at <br />the time of the examination, there is any evidence of myocarditis, coronary sclerosis, <br />or pneumonia or its sequel. In addition to the above, the report shall include a <br />statement as to the applicant's EKG and lung conditions. <br /> <br />SECTION 3. APPLICATION <br /> <br />Every volunteer fire fighter shall be eligible to apply for membership in the Association <br />and shall make written application for membership on a form supplied by the Association <br />not later than 90 days from the date on which the person commenced service as a <br />volunteer fire fighter. Each application shall be accompanied by an entrance fee and <br />shall be referred to the board of trustees for approval. The board shall approve the <br />application for membership if the applicant meets the qualifications set forth in the above <br />provisions of this Article. The board shall act upon the application within 30 days of its <br />submission and within six months from the date on which the application commenced <br />service as a volunteer fire fighter. If the application is rejected, the board shall return the <br />application and entrance fee to the applicant, noting thereon with particularity which <br />qualifications are not met. Thereafter the applicant shall be given the opportunity to <br />appear before the full board within the next 30 days to support a claim for membership. <br />When an application is approved, the application shall receive a certificate of membership <br />signed by the President and Secretary of the Association. <br /> <br />SECTION 4. MEMBERSHIP DUES <br /> <br />Each Association member who is a member of the Fire Department shall be assessed <br />annual dues in an amount of $28.00 per year. The dues shall be payable quarterly, that <br />is, every three months. Any member who fails to pay the quarterly assessment by the end <br />of the quarter following the three-month period for which the dues are owing, shall not be <br />considered a member "in good standing." Notwithstanding any thing herein to the <br />contrary, a member may not be "in good standing" even though that member's, dues are <br />current. Dues paying Association members will be furnished with a wage deduction and <br />authorization form which has been approved by the City and the Association membership. <br />New Association members shall be obligated to pay dues commencing with the month in <br />which the new member was accepted into the Association's membership.
