SCOPE OF SERVICES <br /> <br />The p~oposal shall be submitted in the form of a cont~act <br />agreement ready for execution upon acceptance by the city. The <br />proposal shall be accompanied with a statement of qualifications <br />of the contractor. The contract agreement shall detail all costs <br />including proposed operating budget, terms, and conditions with <br />respect to but not limited to the following items: <br /> <br />1. Provide operation and maintenance activities as required to <br />operate the sanitary sewer and water systems in compliance <br />with all Federal, State and local regulatory requirements <br />including, but not limited to, employment practices, wages, <br />and safety. <br /> <br />2. Contract services shall include, but not be limited to, the <br />provision of all wages, benefits, salaries, materials, equip- <br />ment, vehicles and supplies. <br /> <br />3. Provide sufficient nurr~er of quali=ied personnel including <br />management, technical and administ~ative personnel who meet <br />required Minnesota certification to ope~ate and maintain the <br />sanitary sewer and water syste~s in an acceptable manner. <br /> <br />~. p~ovide sampling, labo~ato~y testing and analysis, and <br />reporting necessa~y for compliance with Minnesota Dept. of <br />Health, Minnesota Pollution Control Agency and Met~opolitan <br />Waste Cont~ol commission permit requi~ements. <br /> <br />5. Provide details of services to be p~ovided by any and all <br />subcon~~actors. <br /> <br />6.
