16.4 Family and Mcdical L ave <br />6.4 Family and Medical Leave <br />Eligibility <br />Commented [CL1]: <br />Formatted: Heading 2 <br />Formatted: Font: Bold <br />Formatted: Normal <br />To qualify to takefor FMLA leave under this policy, an employee must meet all the following <br />conditions: <br />• The employee must hi -lave worked for the GityCity for at least 12 months (or 52 weeks) <br />prior to the start date of the leave is to commence. The 12 months or 52 weeks do not need <br />to be not have been consecutive; however, the GityCity will not countconsider any service <br />occurring more than seven years before the prior to the employee's most recent hire date, <br />unless the break was due to National Guard or Reserves military service obligation. <br />• The employee must have worked at least 1,250 hours during the 12-month period prior to <br />the date when the leave is requested to commence. An eligible "child," with some <br />exceptions, is under 18 years of age. <br />• An eligible "parent" includes a biological parent or a person who stood in the place of a <br />parent. <br />• "Serious Health Condition" means an illness, injury, impairment, or -physical, -or mental <br />condition that involves one of the following: <br />o Hospital Care: Any period of incapasi ycity or treatment connected with inpatient care <br />(i.e., an overnight stay) in a hospital, hospice, or residential medical care facility.; <br />361Page <br />
