5. Proposers are prohibited from contacting any representatives of the City Council <br />regarding the proposal. <br />III. PROPOSAL CONTENTS <br />1. Title Page <br />Show the proposal subject, the name of the proposer's firm, address, telephone number, <br />name of the contact person, and the date. <br />2. Table of Contents <br />Include a clear indication of the material by section and by page number. <br />3. Identification of Qualifications of Assigned Personnel <br />The ideal firm should have extensive experience in municipal comprehensive planning. <br />a. The name of the person who will be responsible for the management and <br />administration of the comprehensive planning process .with a resume <br />describing that person's experience and qualifications. <br />b. The names and resumes of the professional staff that will be assigned to <br />providing services to the City. <br />c. An organizational chart identifying team members and their areas of <br />responsibility. <br />d. A statement of committing the aforementioned staff to this proposal. <br />e. Background information concerning the firm, including number of years in <br />business under this name and breakdown of personel in the proposing office. <br />f. Information on similar comprehensive plan projects completed by the firm <br />including contacts who can be used for references. <br />4. Proposer's Detailed Approach to the Scope of Services <br />a. The proposal shall address in detailed fashion the approach of the firm. <br />5. Basis for Compensation <br />a. The proposal shall include a detailed cost estimate for completing the project. <br />Hourly costs for additional time should also be provided. <br />6. Potential Conflicts <br />a.
